Abstract
In recent years, there are many developments in the surgical repair of cleft lips.

Final goals of cleft lip repair are obtaining functional and cosmetic improvements.

For the best functional and cosmetic results, we are using orbicularis oris muscle flaps in cleft lip repair because of the following advantages;

1) to promote normal lip development and causing the least number of secondary deformities with accurate muscle rearrangement.

2) Elevation of depressed side of cleft lip and reconstruction of the philtral column.

3) It can be used in all types of lip incisions without disturbing the final outside suture line.

4) Need of secondary repairs maybe greatly reduced.
